How to activate your card and view your balance
How can I activate my Health+ by Medica Card? >

There are three ways to do it:

Go to MyBenefitsCenter.com and enter your 16-digit card number in the space provided.

Call 1-833-684-8472 and follow the prompts to enter your card number.

Use the OTC Network app (Android/iOS).
How can I see how much is left on my card or what my benefit allowances are? >

Once you activate your card, you can see the updated total of your benefit allowances in three ways:

Visit MyBenefitsCenter.com and enter your Health+ by Medica card number.

Use the OTC Network app.

Call the card activation and balance line at 1-833-684-8472.

What to know before you start
When I use my Health+ by Medica card, will I still need to submit paper forms to get reimbursed? >

Typically, no. There are only two instances when you’d need to use paper reimbursement forms:
-
If your Health+ by Medica card gets rejected at the point of sale for an eligible dental or eyewear purchase.

If you forget to use your Health+ by Medica card while paying for eligible eyewear or dental services and want to be reimbursed for the charge.
What happens if I forget to use my Health+ by Medica card on a purchase that should have been covered? >

If you pay for dental services or eyewear, you can still submit a paper reimbursement form to recover the expenses under your Health+ by Medica card benefits. You can get a paper reimbursement form by calling the number on the back of your Medica ID card.

If your plan covers OTC, please note: Over-the-counter (OTC) purchases aren’t able to be reimbursed, so remember to use your card for them.
Can my Health+ by Medica card be replaced if it’s lost/stolen/damaged? >

Yes. Just call the number on the back of your Medica ID to ask for a replacement card.
Do funds on my Health+ by Medica card expire? >

Your eyewear and dental benefit allowances are available starting on your effective coverage date. But please note: The balances don’t roll over from year to year.

Only for Medicare Advantage individual plan members: OTC benefit allowances get loaded onto your card quarterly at the beginning of the month in January, April, July, and October, but those balances don’t roll over each quarter.

Why doesn’t my card work everywhere OTC items are sold? >

You can only use your OTC allowance at retailers that participate in the OTC Network. Here’s how to find a participating retailer:

Feel like your purchase was rejected in error? Call the number on the back of your Medica ID card.
Can I use my benefit allowance for OTC items at the same retailers as normal? >

If you’re on a Medicare Advantage individual plan, you can place orders a few different ways:

Go to MyBenefitsCenter.com.
-
Use the OTC Network app.
-
Call Convey OTC at 1-855-858-5937.
-
Shop in store at participating retailers. The full list is on the OTC Network App or at MyBenefitsCenter.com, but they do include Walmart, Walgreens, CVS Pharmacy, Dollar General, and other retailers.

If you’re on a Medicare Advantage group plan or Prime Solution (Cost) group plan (Plan 5 or 11), your over-the-counter benefit allowance is supported through a different program that is not the Health+ by Medica card. You can continue to use your OTC benefits from CVS OTC.

For Group plan members only: You can’t use your Health+ by Medica card for OTC purchases. You’ll find more details on your current CVS OTC benefit in Chapter 4 of your Evidence of Coverage (EOC), or here CVS.com/Benefits. If you need a new copy of your EOC, call the number on the back of your Medica ID card to request one be sent to you.
Where can I use my eyewear and dental benefit allowances? >

You can use your allowance at any eyewear or dental provider in the U.S. that accepts debit cards.

Services must be provided where the primary business is dental care or eyewear.

You’ll need to get dental services from a licensed dentist within the U.S. and its territories. Cosmetic procedures, teeth whitening and orthodonture treatments aren’t covered. Please refer to your Evidence of Coverage (EOC) for more coverage details. If you need a new copy of your EOC, call the number on the back of your Medica ID card to request one be sent to you.

You’ll need to buy eyewear from an eyewear location or freestanding eyewear center in the U.S or its territories.

The provider must accept our Health+ by Medica card. If you visit an eyewear center or dentist that doesn’t participate in our network, call the number on the back of your Medica ID card. You can let us know in case we can add them to our network in the future.
When asked for my ZIP code, what should I enter? >

Enter your five-digit ZIP code of the address registered with Medica (not a temporary address).
When I swipe my Health+ by Medica card at the register, should I select debit or credit? >
-
Use the credit option. And skip the PIN entry because you won’t have a PIN associated with your card.
Can I add my card to my mobile wallet for Apple Pay or Google Pay? >

That feature isn’t available right now. But you can use the OTC Network app’s scan-to-pay feature.
Where can I order a catalog to see items I can use for my OTC benefit allowance as a Medicare Advantage individual plan member? >
What if a transaction’s amount is more than the funds on my card? >

Use your card, then pay for the remaining balance with another form of payment.

What I should know after I purchase?
How do I complete a return? >

Return the item to the retailer where you bought it. The credit will show up on your Health+ by Medica card balance.
What if I notice fraudulent or inaccurate charges on my card? >

Call the phone number on the back of your Medica ID card right away.
